I have been so blessed to have had the opportunity to care for you all these years and it has been a wonderful 40-year journey. So it is with a heavy heart that I announce my intent to retire at the end of this year. We will continue to care for all your medical needs in our office through December 11, 2026. This will include annual preventative exams, same day urgent care visits, referrals and your prescription needs.

If you will need prescription refills and it has been over a year since you have been seen you will need to schedule a visit in our office. This will assure the refills will be available at your pharmacy during this transition. Please call our office soon to schedule these visits as availability may become limited in November and December.

To ensure adequate follow-up we will not provide orders for routine non-urgent labs, imaging or specialist referrals after November 30,2026, as some of this testing can take a week or two to complete and we cannot be assured that we will have confirmation for follow-up prior to our closing.

Your continued care has been our utmost concern, so we have been diligently working with Fanno Creek Clinic to welcome patients and Fanno Creek Clinic has agreed to assume custodianship of the patient records after our office closing. You may go to any clinic you choose and ask them to send a signed authorization release form to our office for your medical records prior to December 11, 2026.

Please call our office if any health issues arise, referrals are needed or if you have any other outstanding health concerns over the next few months.
